Patient Registration
Most patients receiving services at King’s Daughters will touch base with our Patient Registration Team.
Depending on the service and location, you may visit the department or we may handle the most of the registration process over the phone.
During registration, we will collect important information that will help identify you, ensure that we have correct information to bill your insurance on your behalf, and take care of required paperwork, including consent to bill your insurance and consent for treatment.
Patient Registration is located on the first floor of the Center for Advanced Care and is open from 9 a.m. to 8 p.m., Monday through Friday, and 6:30 a.m. to 3 p.m. on Saturday.
When the department is closed, patients are registered for outpatient services in the Emergency Department.
In order to ensure a smooth process, please bring the following with you when you register:
- Social Security Card
- Photo ID, such as Driver’s license, state-issued ID card, Passport
- Any insurance cards
- A copy of the physician’s orders (unless order was submitted electronically)
- A copy of your living will, if you have one
Persons under the age of 18 must be accompanied by a parent or legal guardian.
We also offer a pre-registration service, which allows patients to register for outpatient services by telephone. Pre-registration hours are 8 a.m. to 5 p.m., Monday through Friday.
Patients who pre-register for services are asked to bring their insurance cards, physician order, and photo ID on the day of service.
