MyChart: Your Records Where,
When You Want Them
Your medical record is maintained electronically at King’s Daughters. We use an electronic medical record (EMR) system called Epic, which allows physicians and providers associated with KDMC to have access to your medical record wherever you are being cared for.
The Epic system integrates information from anywhere you receive treatment at King’s Daughters. It allows physicians and caregivers to have a complete clinical profile, in a very secure, state-of-the-art system.
An important benefit of the EMR is that our patients have free secure online access to portions of your EMR where your physician stores your health information. It’s your record, and access is secure and completely free.

- Send an appointment request to your provider 24/7/365
- Request prescription renewals
- Get test results
- Keep track of your health, including current issues, medications, growth charts, allergies, immunizations and medical, surgical and family history.
To sign up for MyChart, contact your physician’s office and request your special access letter. The letter will provide you with a password and instructions on setting up your MyChart account.
How to Get Copies of Films and Other Records
If you need to get copies of CT, MRI or other imaging studies, please call the KDMC Film Library at (606) 408-0746 at least 24 hours in advance. You will be asked to complete a release form and to bring a valid driver’s license or other form of state-issued ID to pick up the films.
For copies of your medical record, please call our Medical Records Department Monday through Friday 7am - 6pm at (606) 408-1815. You will need to complete the Consent to Release Information form. Depending upon the records you are requesting, it may take up to 21 days to process your request.
